The trapezius is a large, triangular muscle spanning from the base of the skull down to the mid-back and out to the shoulders. What Happens on the Day
The appointment itself is short. You sit upright while Dr Azra asks you to shrug against resistance, which makes the upper trapezius stand out and shows exactly where the bulk sits and how far down the muscle it extends. She marks the injection points on both sides, comparing left with right — most people are noticeably stronger on their dominant side, and matching the dose to the muscle rather than splitting it evenly is what keeps the result symmetrical.
The injections themselves take around ten minutes. The needle is fine and the sensation is a brief pinch at each point; the trapezius is a thick muscle, so the needle goes deeper than it would in the face, and some patients describe a dull ache for a few seconds afterwards. No anaesthetic is normally needed and no numbing time is required, which is why the whole visit fits inside 15–20 minutes.
You can drive yourself home and return to work immediately. There is no dressing, no swelling to hide, and nothing about the treated area looks different on the day — the change is in what the muscle does over the following weeks, not in how it looks that afternoon. Aftercare — the First 48 Hours
Trap Tox aftercare is simple, and almost all of it concerns the first two days while the toxin is binding to the nerve endings in the muscle:
After 48 hours there are no restrictions at all — normal training, massage and posture work can resume, and physiotherapy in particular tends to become easier once the muscle stops over-recruiting. - No heavy lifting or shoulder-intensive exercise for 48 hours — that includes gym pressing, pull-ups, carrying shopping on one shoulder and heavy bags.
- Do not massage or rub the treated area for 24 hours, and skip a deep-tissue or sports massage across the shoulders for the first few days.
- Stay upright for four hours and avoid lying face-down or napping straight after the appointment.
- Skip saunas, steam rooms and hot yoga for 24 hours. Heat increases circulation through the area and is best left until the following day.
- Expect mild tenderness. The injection points can feel like a post-workout ache for a day or two, which is normal and settles on its own.
Risks, Side Effects and Safety
Trapezius Botox uses the same botulinum toxin, in the same way, as any other Botox treatment — the difference is the size of the muscle and therefore the dose. The common side effects are local and short-lived: small bruises at the injection points, tenderness for a day or two, and occasionally a mild headache in the first 24 hours. Bruising is more likely if you take blood thinners, fish oil or anti-inflammatories in the days beforehand.
The effect that patients ask about most is strength. Because the trapezius is deliberately weakened, you may notice that shrugging feels less powerful for the first few weeks. Everyday activity, overhead reaching and normal gym work are not meaningfully affected, because the shoulder girdle is driven by several muscles working together and only one of them has been treated. If you train heavily with overhead or trap-dominant lifts, say so at the assessment — the dose can be planned around it.
Less commonly, an uneven result can occur if one side responds more than the other. It is correctable with a small top-up two to three weeks after the first session, which is why a review is built into the treatment rather than offered as an extra. Botulinum toxin is not given in pregnancy or while breastfeeding, and it is avoided in neuromuscular conditions such as myasthenia gravis.
