How Dr Azra Decides Between Them

A face that looks tired rarely has one thing wrong with it. Ageing works on four separate layers at once — bone resorbs and the facial skeleton narrows, deep fat pads shrink and slide downward, the ligaments holding everything in place stretch, and the skin itself loses collagen, elastin and water. A treatment that addresses one layer will not fix a problem that lives in another, which is why two people with the same complaint can leave with entirely different plans.

The assessment is therefore structural before it is cosmetic. Dr Azra looks at the face at rest and in movement, from the front and in profile, and works out which layer is driving the change. Volume loss in the mid-face is treated at the mid-face even when the patient points at the nasolabial fold, because filling the fold itself only makes the heaviness worse. Sagging that comes from ligament laxity is lifted, not filled. Lines that only exist during expression are relaxed rather than filled, and lines that are etched in at rest usually need both.

Age is a rough guide rather than a rule. In the late twenties and thirties the work is mostly preventive and skin-quality led — boosters, light Botox, occasional small-volume filler. Through the forties, volume replacement and collagen stimulation start doing the heavy lifting. From the fifties onward, lift becomes the limiting factor and threads or a combination approach matter more than any single injectable. Budget and downtime are part of the clinical decision, not separate from it. A plan that a patient cannot maintain is a worse plan than a smaller one they will keep up with, so treatments are often sequenced across several months rather than delivered in one long appointment.

When Results Actually Appear

Non-surgical does not mean instant. Different treatments run on completely different clocks, and knowing which clock you are on prevents both disappointment at week two and unnecessary top-ups at week three.

Immediately after

Fillers show most of their result on the table, though 10–30% of what you see on day one is swelling. Botox shows nothing yet. Threads give a visible lift straight away that will settle slightly. Boosters and RF look no different, and may look temporarily worse.

Days 3 to 14

Swelling and bruising resolve. Botox begins working at day 3–5 and reaches its full effect at day 10–14, which is when a review appointment is useful. Filler softens into the tissue and looks less “placed”. Thread tightness eases.

Weeks 4 to 12

This is when collagen-driven treatments arrive. Biostimulators, RF microneedling and threads all work by provoking new collagen, and new collagen takes six to twelve weeks to become visible. Judging these treatments earlier than four weeks is the most common mistake patients make.

Safety, Suitability and Who Should Wait

Every treatment on this page is a medical procedure, and a consultation exists partly to find the people who should not have one yet. Full medical history, current medication and previous aesthetic work are taken before anything is planned.

Absolute reasons to postpone

Active infection or inflamed acne in the treatment area, active cold sores, current pregnancy or breastfeeding, and any known allergy to a component of the product. Injectables are also deferred while a bacterial or dental infection is being treated.

Conditions that need a conversation first

Autoimmune and connective-tissue disease, current isotretinoin, anticoagulants, a history of keloid scarring, and previous permanent or unidentified filler elsewhere in the face. None of these is automatically disqualifying — they change product choice, depth and timing.

Risks worth knowing

Bruising, swelling and tenderness are expected and self-limiting. Lumps, asymmetry and over-correction are usually correctable, and hyaluronic acid filler can be dissolved. Vascular occlusion is rare but is the reason injectables belong with a physician who can manage it.

Aftercare, in short. For 24 hours after any injectable: stay upright for the first four hours, skip the gym, alcohol and hot showers, and do not massage the area unless you have been told to. For two weeks: no sauna, hammam or facial treatments over the treated zone, and postpone dental work where possible. After RF microneedling or Dermapen, the skin is an open barrier for roughly 24 hours — bland moisturiser, no actives, and daily SPF 50 for a fortnight.
