Botox Aftercare Guide
Following proper aftercare ensures your Botox settles correctly, reduces the risk of bruising, and produces the best possible result. This complete guide from Dr Azra Vaziri covers everything from the first 4 hours through long-term maintenance — for every treatment area. First 4 Hours (Critical Window)
The first 4 hours after injection are the most important. Botox needs time to bind to nerve endings at the injection site before it can take full effect. During this window:
- Stay upright — do not lie down, bend over, or lean forward for at least 4 hours. This prevents the toxin from migrating away from the intended muscles.
- Do not touch, rub, or massage the treated areas. Pressure can push Botox into adjacent muscles and cause unintended effects.
- Gently exercise the treated muscles — frown, raise your eyebrows, squint, smile. This gentle contraction helps the toxin bind to motor nerve endings faster.
- Avoid hats, headbands, helmets — anything that creates pressure on the treated zone.
- No makeup for at least 4 hours. Applying and removing makeup involves touching and pressure on the skin.
First 24 Hours

First Week
After the first 24 hours, most restrictions ease. Here’s what to know for the rest of the week:
Exercise: You can resume normal exercise after 24 hours. Avoid inverted positions (headstands, heavy bending) for 48 hours. High-intensity exercise is fine from day 2.
Skincare: Resume your full skincare routine after 24 hours, including retinoids, vitamin C, and AHAs. Avoid aggressive exfoliation or chemical peels on treated areas for 3–5 days. Makeup: Full makeup application can resume after 12 hours. Use clean brushes and be gentle over injection sites. You may notice tiny red dots at injection points — these fade within hours to a day and can be concealed.
Facials and other treatments: Wait at least 2 weeks before any facial treatment that involves massage, pressure, microcurrent, LED, or heat on the treated area. Microneedling, RF microneedling, and laser should be scheduled at least 2 weeks before or after Botox.
Dental work: Wait 2 weeks before extensive dental procedures that require prolonged mouth opening — particularly relevant after masseter Botox.

Managing Bruising, Swelling & Pinpoint Marks
Botox uses very fine needles, so most people have little more than a few tiny marks. When mild reactions do happen, they are normal and short-lived. Here is what to expect and how to settle each one.
Bruising
Small bruises can appear where a tiny vessel was nicked, most often around the eyes. They fade over 5–7 days and can be concealed with makeup after 12 hours. A cool compress in the first hours and avoiding alcohol, aspirin, and ibuprofen for 24 hours reduce the risk.
Swelling & small bumps
Tiny raised bumps at the injection points are the injected fluid and usually settle within a few hours. Mild swelling — more common with hyperhidrosis or masseter areas — resolves within a day. Sleep slightly elevated the first night.
Pinpoint marks & redness
Tiny red dots at each entry point are normal and fade within a few hours to a day. Leave them makeup-free for the first 4 hours, then conceal gently with a clean brush if needed.
Mild headache
A mild headache in the first day or two is occasionally reported, particularly after forehead or frown-line treatment, and usually eases on its own. Paracetamol is fine — avoid aspirin and ibuprofen, which can add to bruising.
Temporary heaviness
A slight heaviness in the brow, especially after forehead Botox, can appear as the muscles adjust and settles within the first week as the result balances out.
Speeding recovery
Arnica cream or tablets, staying hydrated, and keeping the head elevated all help marks clear faster. Book important events at least 2 weeks after treatment so any bruising has fully resolved.

Area-Specific Aftercare Tips

Forehead Botox
Avoid tight hats or headbands for 24 hours. Raise your eyebrows periodically in the first few hours. A slight brow heaviness at first settles within a week as the muscles adapt.
Frown Line Botox
Try frowning repeatedly in the first few hours to help uptake. The “11” lines between the brows should begin softening within 3–5 days.
Crow’s Feet Botox
Avoid rubbing your eyes. Be gentle when removing eye makeup for the first 48 hours. Squint and smile to exercise the muscles.
Masseter Botox
You can eat normally immediately — no dietary restrictions. Jaw slimming develops over 4–8 weeks as the muscle atrophies. If you grind, you may notice less grinding within the first week.
Lip Flip Botox
Avoid using straws for 24 hours. Eating and drinking are fine. The upper lip may feel slightly different for a few days.

Warning Signs — When to Contact the Clinic

- Drooping of an eyelid or eyebrow, or noticeably uneven results that appear after the first few days
- Difficulty swallowing, speaking, or breathing (rare — seek urgent medical care)

- Severe or spreading swelling, or swelling that worsens after 48 hours
- Signs of infection at an injection site: increasing redness, warmth, pus, or fever
- A headache that is severe or does not settle with paracetamol
