Botox Aftercare Guide

Following proper aftercare ensures your Botox settles correctly, reduces the risk of bruising, and produces the best possible result. This complete guide from Dr Azra Vaziri covers everything from the first 4 hours through long-term maintenance — for every treatment area.

First 4 Hours (Critical Window)

The first 4 hours after injection are the most important. Botox needs time to bind to nerve endings at the injection site before it can take full effect. During this window:

  • Stay upright — do not lie down, bend over, or lean forward for at least 4 hours. This prevents the toxin from migrating away from the intended muscles.
  • Do not touch, rub, or massage the treated areas. Pressure can push Botox into adjacent muscles and cause unintended effects.
  • Gently exercise the treated muscles — frown, raise your eyebrows, squint, smile. This gentle contraction helps the toxin bind to motor nerve endings faster.
  • Avoid hats, headbands, helmets — anything that creates pressure on the treated zone.
  • No makeup for at least 4 hours. Applying and removing makeup involves touching and pressure on the skin.

First 24 Hours

✓ Do
  • Keep treated area clean
  • Drink plenty of water
  • Take paracetamol if needed (not aspirin/ibuprofen)
  • Sleep on your back
  • Resume gentle skincare after 12 hours
  • Continue gentle facial exercises
✗ Avoid
  • Exercise or strenuous activity
  • Alcohol (increases bruising risk)
  • Saunas, steam rooms, hot yoga, hot showers
  • Facials, facial massage, or microcurrent
  • Ibuprofen or aspirin (blood thinners)
  • Sleeping face-down or on your side

First Week

After the first 24 hours, most restrictions ease. Here’s what to know for the rest of the week:

Exercise: You can resume normal exercise after 24 hours. Avoid inverted positions (headstands, heavy bending) for 48 hours. High-intensity exercise is fine from day 2.

Skincare: Resume your full skincare routine after 24 hours, including retinoids, vitamin C, and AHAs. Avoid aggressive exfoliation or chemical peels on treated areas for 3–5 days. Use SPF 50 daily — UV damage degrades results faster. For a broader overview of line and wrinkle options, see wrinkle treatment Dubai.

Makeup: Full makeup application can resume after 12 hours. Use clean brushes and be gentle over injection sites. You may notice tiny red dots at injection points — these fade within hours to a day and can be concealed.

Facials and other treatments: Wait at least 2 weeks before any facial treatment that involves massage, pressure, microcurrent, LED, or heat on the treated area. Microneedling, RF microneedling, and laser should be scheduled at least 2 weeks before or after Botox.

Dental work: Wait 2 weeks before extensive dental procedures that require prolonged mouth opening — particularly relevant after masseter Botox.

Area-Specific Aftercare Tips

The general aftercare rules above apply to all areas. Here are additional tips specific to each treatment zone:

After forehead Botox: Avoid wearing tight hats or headbands for 24 hours. Raise your eyebrows periodically in the first few hours. You may notice a slight heaviness in the brow area initially — this settles within a week as the muscles adapt.

After frown line Botox: Try frowning repeatedly in the first few hours to help uptake. The “11” lines between the brows should begin softening within 3–5 days.

After crow’s feet Botox: Avoid rubbing your eyes. Be gentle when removing eye makeup for the first 48 hours. Squint and smile to exercise the muscles.

After masseter Botox: You can eat normally immediately — no dietary restrictions. The jaw slimming effect develops gradually over 4–8 weeks as the muscle atrophies. If you have bruxism, you may notice reduced grinding within the first week.

After lip flip Botox: Avoid using straws for 24 hours. Eating and drinking are fine. The upper lip may feel slightly different for a few days as you adjust. Avoid exfoliating lip products for 48 hours.

After Nefertiti lift: Avoid necklaces or tight scarves that press on the jawline and neck area. Neck movements are fine — just avoid rubbing the injection sites.

After bunny lines Botox: Avoid pressing or scrunching your nose for 24 hours. Results in this area are typically visible within 3–5 days.

After gummy smile Botox: Smile normally. The effect develops over 3–7 days. Eating and drinking are unaffected.

After hyperhidrosis (sweating) Botox: Keep underarms or palms clean and dry. Avoid applying deodorant or antiperspirant for 12 hours. Sweat reduction typically begins within 2–5 days. No exercise for 24 hours.

After trap tox (trapezius Botox): Avoid heavy lifting or shoulder-intensive exercise for 48 hours. The shoulder slimming effect develops gradually over 4–8 weeks as the trapezius muscle relaxes and atrophies.

After baby Botox: Same aftercare as standard Botox. Lower doses wear off faster (2–3 months), so plan your next session accordingly.

After Xeomin or Dysport: Aftercare is identical to Botox. Dysport may show results slightly faster (2–3 days). Compare brands →

Timeline: When to Expect Results

Day 1–3: You may not notice anything yet. This is completely normal — Botox needs time to block the nerve signal.

Day 3–7: Effects begin to appear. Muscles start to relax. Movement reduces. Lines begin softening.

Day 7–14: Full effect develops. This is when to properly assess your results — not before.

Week 2–4: If you notice asymmetry or want a slight adjustment, schedule a follow-up with Dr Azra. Small touch-ups are sometimes needed at the 2-week mark.

Month 3–6: Effects gradually wear off as nerve function returns. Plan your next session before full movement returns to maintain the conditioning effect. See our units guide for dosing at each area, and the Botox price guide for Dubai costs.

How to Make Your Botox Last Longer

  • Stay consistent with your treatment schedule — regular Botox conditions muscles over time, which can extend the duration of each session.
  • Use SPF 50 daily — UV damage breaks down collagen and elastin, which accelerates muscle-related ageing and can make Botox wear off faster.
  • Stay hydrated — well-hydrated skin responds better to all aesthetic treatments.
  • Avoid excessive heat exposure — saunas, prolonged sun, and hot environments increase metabolic rate and may speed up Botox breakdown.
  • Consider zinc supplementation — some studies suggest zinc may enhance Botox effectiveness and duration, though evidence is still limited.
  • Combine with skin quality treatments — Profhilo, skin boosters, and preventative Botox strategies create a comprehensive anti-ageing approach that supports Botox results.
  • Don’t wait too long between sessions — re-treating before full movement returns means muscles never regain full strength, which leads to longer-lasting results over time. See Botox myths vs facts for more on long-term use.

Frequently Asked Questions

Wait at least 24 hours before strenuous exercise. Light walking is fine the same day. Increased blood flow and body heat from exercise can cause Botox to migrate before it has fully bonded to nerve endings.

Stay upright for at least 4 hours. Sleep on your back that night. Avoid sleeping face-down for 24–48 hours to prevent pressure on treated areas.

Wait 4–6 hours minimum. Ideally leave treated areas makeup-free for 12 hours. When applying, use clean brushes and dab gently rather than rubbing.

Avoid alcohol for 24 hours. It acts as a blood thinner and increases bruising risk at injection sites.

Results begin appearing in 3–7 days. Full effect at 2 weeks. If you don’t see the desired result by 2 weeks, contact Dr Azra for a follow-up.

Wait until 2 weeks before assessing symmetry — one side may develop faster. If asymmetry persists after 2 weeks, a small touch-up injection can correct it.

Wait at least 2 weeks before any facial involving massage, pressure, microcurrent, or heat. Gentle cleansing and hydrating skincare can resume after 12 hours.

Use SPF 50 daily, stay hydrated, avoid excessive heat, maintain a consistent treatment schedule, and don’t wait until full movement returns before re-treating. Regular Botox conditions muscles over time for longer-lasting results.
