Subcision for Acne Scars in Dubai: How It Works

Subcision is a minor in-clinic procedure that treats depressed acne scars — particularly rolling scars — by releasing the fibrous bands that tether the scar down to the tissue beneath. Once those bands are cut, the skin is free to lift back to its normal level, and the controlled injury stimulates fresh collagen to fill the space from below.

What Is Subcision?

Subcision — short for subcutaneous incisionless surgery — is a technique in which a fine needle or blunt cannula is passed just under a depressed scar, moving in a gentle fanning motion to cut the fibrous strands anchoring the scar to the deeper layers of skin. These tethering bands are what pull a rolling scar downward and create the shadowed, wave-like dip you see on the surface.

Two things happen once the bands are released. First, the skin is mechanically freed and rises toward the level of the surrounding tissue. Second, the micro-trauma of the procedure triggers the body’s wound-healing response, laying down new collagen in the released pocket that supports the lift over the following weeks. It is this dual action — immediate release plus gradual collagen fill — that makes subcision so effective for scars that resurfacing treatments cannot reach.

It is one of the few acne scar treatments that addresses the cause of a depressed scar rather than only resurfacing the top of the skin. For that reason subcision is often the turning point for rolling and tethered scars that have not improved with microneedling or peels alone.

Because it works beneath the surface rather than on top of it, subcision is safe across all Fitzpatrick skin types, including the deeper skin tones common in the UAE, with a low risk of the pigmentation changes that can follow more aggressive laser resurfacing.

Not sure whether your scars are tethered? Book a scar assessment and each scar is examined and mapped individually.

At her Dubai clinic, Dr Azra Vaziri performs subcision as one stage of a personalised acne scar treatment plan, usually combined with RF microneedling.

Which Scars Does Subcision Treat?

Well suited to subcision

  • Rolling scars — the classic indication, caused by tethering bands
  • Broad, shallow depressions that move with the skin
  • Tethered boxcar scars with a bound-down base
  • Depressed scars that have plateaued with microneedling alone

Less suitable — better options exist

Unsure which type you have? Book a scar assessment — scar type decides the treatment, not the other way round.

  • Ice pick scars — too narrow and deep; TCA CROSS is preferred
  • Raised or keloid scars — subcision is for depressed scars only

  • Flat pigmentation or redness — treated with peels, not subcision
  • Active, inflamed acne — must be controlled first

What Happens During Subcision

Ready to have your scars mapped? Book a scar assessment in Dubai.

  1. Assessment & mapping — Dr Azra Vaziri examines your scars in raking light, identifies which are tethered, and marks each one. Only depressed, bound-down scars are selected for subcision.
  2. Numbing — the area is cleaned and local anaesthetic is injected so the procedure itself is comfortable. You stay awake throughout.
  3. Band release — a fine needle or blunt-tip cannula is introduced through a single entry point and moved in a fanning sweep beneath each scar, freeing the fibrous tethers. You may feel pressure but not sharp pain.
  4. Support (optional) — where useful, RF microneedling, a small amount of biostimulator, or filler is added so the newly released skin is held up while collagen forms.
  5. Aftercare — pressure is applied to limit bruising, and you leave with clear aftercare guidance. The whole visit typically takes 30 to 45 minutes.

Recovery & downtime

Expect bruising and swelling at the treated sites — this is normal and part of how subcision works. Bruising usually settles over 5 to 14 days and is easily covered with makeup after the first day or two.

Most people return to work the same or next day. Strenuous exercise and alcohol are best avoided for 48 hours to limit bruising, and sun protection is essential throughout — important in the UAE’s strong daylight.

Results & timeline

Some lift is visible immediately once the bands are released, but the real improvement builds over 6 to 12 weeks as new collagen fills the space. Most rolling-scar patients need 2 to 3 sessions spaced 4 to 6 weeks apart for a full result.

Results are long-lasting because the tethering bands are physically released. Combining subcision with RF microneedling typically produces a smoother, more complete result than either treatment alone — ask about a combined plan.

Subcision Is Usually Part of a Combination

Subcision releases the scar; these treatments fill and refine it.

The right combination depends on your scar mix — see the full acne scar treatment guide, or book a scar assessment.

Important to know

If anything concerns you after a session, contact the clinic — you are reviewed between sessions.

  • Bruising is expected, not a complication — it reflects the release that makes subcision work. Plan around any events for a week or two afterwards.

  • Active acne must be under control first — treating tethered scars over inflamed skin risks new scarring.
  • Occasionally a small firm nodule can form as it heals; it is usually self-limiting and manageable at review.
  • Realistic expectation: significant improvement over a course, not total erasure. Subcision is powerful but works best in combination.

Why Patients Choose Dr Azra for Subcision

Subcision is a technique-sensitive procedure: the result depends on correctly identifying which scars are genuinely tethered, releasing them at the right depth, and supporting the freed skin while new collagen forms. Performed too superficially it disappoints; performed by an experienced hand it is often the single biggest step-change in a rolling-scar journey. At her Dubai clinic, Dr Azra Vaziri treats subcision as one stage of a mapped scar plan rather than a stand-alone fix, and reviews progress between sessions so the approach adapts to how your skin actually responds.

Consultations and treatment are available at the Dubai clinic.

  • Doctor-led assessment — each scar is examined in raking light and only genuinely tethered scars are selected, so you are not treated for scars subcision cannot help.
  • Combination expertise — subcision is planned alongside RF microneedling, biostimulator or filler in one protocol, so deep structure and surface texture are treated together.
  • Aftercare and review — you leave with clear guidance and are seen again to check healing, manage bruising and plan the next session for the best cumulative result.

Whose Hand Performs Your Subcision — Dr Azra Vaziri

Subcision is done blind, under the skin, by feel: the cannula has to find the tether and release it at the right plane without shearing what is around it.

Every Subcision for Acne Scars appointment at this clinic is assessed and injected by Dr Azra Vaziri herself — a DHA- and DOH-licensed aesthetic physician with more than 20 years of experience, practising aesthetic medicine since 2004.

Frequently Asked Questions

What is subcision for acne scars?

Subcision is a minor in-clinic procedure that treats depressed acne scars by passing a fine needle or cannula beneath the scar to cut the fibrous bands tethering it to deeper tissue. Releasing these bands lets the skin lift, and the controlled injury stimulates new collagen. It is the most effective single technique for rolling scars and is usually combined with RF microneedling or filler.

Which acne scars does subcision work best on?

Subcision works best on tethered, depressed scars — especially rolling scars and bound-down boxcar scars that move with the skin. It is not suitable for narrow ice pick scars (treated with TCA CROSS) or for raised and keloid scars, which are not depressed.

Is subcision painful?

Local anaesthetic is injected first, so the procedure itself is comfortable — most patients feel pressure and movement rather than sharp pain. Mild tenderness and bruising afterwards are normal and settle within one to two weeks.

How much downtime does subcision need?

Bruising and swelling are expected and typically last 5 to 14 days, though most people return to work the same or next day and cover any bruising with makeup. Avoid strenuous exercise and alcohol for 48 hours, and protect the area from the sun while it heals.

How many subcision sessions will I need?

Most patients with rolling scars need 2 to 3 sessions spaced 4 to 6 weeks apart. Improvement is progressive — some lift is immediate, but the fuller result develops over 6 to 12 weeks as new collagen forms in the released pocket.

Is subcision safe for darker skin tones?

Yes. Because subcision works beneath the skin surface rather than resurfacing the top layer, it carries a low risk of the post-inflammatory pigmentation that can follow ablative laser in higher Fitzpatrick skin types. Dr Azra Vaziri tailors every scar protocol to your skin type.

Subcision or RF microneedling for rolling scars?

They do different jobs and work best together. Subcision physically releases the tethering bands that pull a rolling scar down, while RF microneedling rebuilds collagen to fill and firm the released area. For established rolling scars, combining the two in one plan gives a more complete result than either alone.

How much does subcision cost in Dubai?

Subcision is priced according to the number of scars treated and whether it is combined with RF microneedling, filler, or biostimulator in the same session, so it is quoted after a scar assessment. See the acne scar treatment cost page for the wider price list, or book a consultation for a personalised plan.
