⎯ Understanding the Treatment

HArmonyCa is a hybrid injectable gel made by Allergan Aesthetics: calcium hydroxyapatite microspheres suspended in a cross-linked hyaluronic acid gel, with lidocaine already mixed in. It lifts immediately like a filler and stimulates collagen like a biostimulator.

The two ingredients are not a marketing pairing — they do measurably different jobs. The HA gel is the part that restores contour the moment it is placed. The CaHA microspheres, 25–45 microns across, stay behind as a scaffold that fibroblasts colonise, laying down new type I collagen over the following weeks.

That is the difference from an HA dermal filler, which occupies space and then goes; and from a pure biostimulator like Sculptra, which builds collagen beautifully but shows you nothing on the day. ⎯ The Science

From your twenties you lose roughly one percent of your collagen a year. The visible consequence is not only flatness — it is laxity: the cheek descends, the jawline blurs, a prejowl shadow appears. HArmonyCa was formulated for exactly that pattern.

Injected into the subcutaneous plane, usually with a blunt cannula in threads rather than as a bolus, the HA gel immediately restores support along the treated vector. That is the part you see in the mirror before you leave.

The CaHA microspheres then integrate with the surrounding tissue. Laboratory work shows hybrid HA/CaHA integrates into tissue faster and deeper than CaHA alone, and triggers type I collagen gene expression and protein production. Over weeks, that new collagen thickens the dermis and firms the area — which is why a good HArmonyCa result keeps improving after the swelling has settled.

HArmonyCa is a lower-face and mid-face product. Being honest about where it should not go is as useful as listing where it should — and it is the part most clinic pages leave out.

⎯ What Actually Happens

The whole visit runs 45 to 60 minutes, of which the injecting itself is about half. Nothing is placed until the plan is agreed and photographed.

Individual timelines vary with age, skin quality, how much laxity was present at the start and how many syringes were used.

HArmonyCa is a good product badly matched to a lot of the people who ask for it by name. The two lists below are the ones Dr Azra works through at the consultation.

⎯ Safety

The common effects are the ordinary injection ones: redness, swelling, tenderness, itching and sometimes bruising. Injection-site reactions typically settle within 24–48 hours and swelling within about a week.

Firm nodules are the specific risk people read about with CaHA products. In the published series they are uncommon — under three percent — mostly mild and temporary, and they are largely a technique problem: too superficial, too much in one place, or placed in a hyperdynamic area. Retrograde threads, no boluses, and small volumes in mobile zones are how they are avoided.

The important asymmetry with HA filler: if something needs correcting, the HA part of HArmonyCa can be dissolved with hyaluronidase, but the CaHA part cannot. That is a reason to be conservative on the first session and review at four weeks — which is exactly how Dr Azra runs it.
